Description (Service)

Community based services for people with physical disabilities

Attendant Care Services - non-medical, one-on-one care, assistance with activities of daily living, in home, school or workplace

MODC Non-Profit Housing - supportive care in in five locations around Ontario

Home and Vehicle Modification Program -1-877-369-4867 - financial assistance (up to $15,000) for basic home and vehicle modifications for permanent Ontario residents with a substantial impairment expected to last one year or more

Brain Injury Services - life skills, communication, community integration, emotional and behavioural support, vocational support, and recreation for people dealing with the effects of brain injury

Aphasia and Communication Disabilities Program - group communication program for adults with aphasia and other acquired communication disabilities living in York, Durham and Peterborough regions

Programs for children and youth with disabilities - growing skills, building confidence, uniquely tailored to help children reach new milestones and feel empowered

Paving the Path to Work - employability program for youth and young adults with disabilities to gain the tools needed to turn aspirations into achievable goals

Post-Polio Canada - in-person and virtual support services, resources and information for polio survivors at Post-Polio Canada

After Stroke - afterstroke.ca -- person-to-person support, educational opportunities and community based programs designed to provide connection to people, hope, and life after stroke

Employment Services - assisting people with disabilities find and keep meaningful jobs, matching people who have disabilities with employers seeking qualified job candidates at Employment services
