Nurse Practitioner-led multidisciplinary clinic providing safe, accessible and gender affirming care within a multidisciplinary, shared care model * team includes a Registered Nurse (RN), Registered Practical Nurse (RPN), Social Worker, System Navigator, and Pharmacist

Services include:

• Support for social, medical and legal transition processes
• Education and information on gender identity and healthcare options
• Surgical assessment, referrals and support
• Sexual health education and screening
• Individualized support for hormone therapy and monitoring
• Psychosocial support throughout the transition process
• Group therapy sessions and workshops

Mon-Fri 8am-5pm * After-hours clinic: Mon-Thu 6pm-8pm; Sat 9am-3pm
